A business jet carrying a former White House official this month pitched up and down in midair, causing her death, after pilots turned off a system that stabilizes the plane, the National Transportation Safety Board said in a preliminary report.
The pilots noticed several warnings related to system failures of the aircraftโs flight control as it traveled from Keene, N.H., to Leesburg, Va., on March 3, and they followed steps on a checklist, which advised them to turn off a switch that controls a stabilizer function in the aircraft, according to the report.
When the switch was turned off, it prompted the nose of the plane to swing upward, the report says. The plane then pointed down and jerked upward again in a roller-coaster-like motion before a pilot used both hands to regain control of it, the N.T.S.B. said.
โAs soon as the switch position was moved, the airplane abruptly pitched up,โ the report says. One of the pilots, in his account to investigators, estimated that the plane had oscillated up and down for a โfew seconds.โ
Shortly after the scare, the crew members were told by a passenger that the former White House official, Dana Hyde, 55, of Cabin John, Md., had been injured. Ms. Hyde, who was a senior adviser at the State Department under President Barack Obama and served as counsel on the 9/11 Commission, was taken to a hospital after the plane landed.
She died in the hospital from her injuries, the authorities said.
The people onboard the plane, a Bombardier Challenger 300, were subjected to forces about four times the pull of gravity, according to the report.
The authorities initially said that the plane had encountered โsevere turbulence.โ But the flight crew members told investigators that they โdid not experience any remarkable turbulence during the flight,โ the report says.
The jet was diverted to Bradley International Airport in Windsor Locks, Conn.
Last year, the Federal Aviation Administration issued a directive to pilots flying the Bombardier Challenger 300 that instructed them to conduct extra safety tests before flights. The directive was prompted by several reports that the horizontal stabilizer caused the noses of planes to tilt down when pilots tried to make the plane climb, according to a rule from the F.A.A. in June last year.
Bombardier said in a statement on Sunday that it was โdeeply saddened by this tragic event,โ adding, โWe extend our sincerest sympathies to all those affected by this accident.โ
Bombardier said it was โcarefully studyingโ the preliminary report from the N.T.S.B.
Ms. Hydeโs husband, Jonathan Chambers, said in a statement that they and their younger son had been flying home that day, after visiting schools in New England, when their plane โsuddenly convulsed in a manner that violently threw the three of us.โ
โDana was the best person I ever knew,โ he said, adding that she โnever forgot her small-town eastern Oregonian roots.โ
Ms. Hyde was a special assistant to the deputy attorney general in the Clinton administration, according to Columbia University.
She later served as associate director of the Office of Management and Budget during the Obama administration. She also helped establish the African Leadership University and worked at Jerusalem Venture Partners, a venture capital firm. โThatโs a real rรฉsumรฉ,โ her husband said.
Mr. Chambers said his wife was โa wonderful mother to our boys.โ
In a eulogy for Ms. Hyde delivered by Mr. Chambersโs older son, he dismissed the idea of living each day as if it were your last.
โI now realize thatโs wrong,โ the son said, according to Mr. Chambers. โYou should live each day as if itโs the last of someone you love.โ

A few train cars have derailed into the Mississippi River in Wisconsin, according to authorities. There appear to be no fatalities, but the extent of injuries are unknown.

Four BNSF railroad crew members were transported to a nearby hospital for medical examinations after a southbound freight train derailed in southwestern Wisconsin, sending some rail cars into the Mississippi River.
Residents of De Soto said it was the "loudest shriek they ever heard" when a BNSF Railway freight train derailed shortly after noon Thursday between De Soto and Ferryville in Crawford County.
The cause of the derailment is still unknown. Two locomotives and an unknown number of cars derailed on the eastern side of the river, a BNSF spokesperson said in a statement.
Crawford County Emergency Management Director Jim Hackett said the derailment poses no immediate threat to public safety.
Hackett said the derailed train cars contained paint, oxygen and lithium batteries.
"At this time, we're not concerned about any hazard to the public, the batteries have not gone into the water," he said.
A Vernon County hazmat team is on site in the event of a hazardous waste situation. If lithium is exposed to water, a rupture or explosion can occur.
Two of the freight train's three locomotives derailed and a diesel fuel leak from one of the locomotives has been "fairly contained" to the river bay, said Brandon Larson, director of Vernon County Emergency Management and Hazmat.
Larson's team has deployed absorbent booms to soak up the spillage.
Two cars holding painted floated south down the river. Hackett said boat crews were able to secure the rouge cars to the bank.
Nearly 30 state, local and public agencies responded to the incident. Hackett said the community response โ many volunteer firefighters โ was "quick" and "amazing."
First responders are expected to work through the night at the site. Officials said they do not know how long the clean up process will take.
BNSF is working to build "access ways" of rock and sand from Highway 35 to the railroad that runs parallel to the highway. Due to the waterway in between the railroad and the highway, a platform must be constructed so crews can access and remove the cars.
Private truck drivers contracting with BNSF have been hauling sand and rock from a quarry in Genoa to the accident location.
The cause of the derailment is under investigation, said BNSF spokesperson Lena Kent said. She did not immediately respond to a follow-up email inquiring about whether whether the derailment had caused any environmental contamination.
The Federal Railroad Administration, a division of the U.S. Department of Transportation that regulates safety across the nation's railroads, said it was sending a team to the site to gather information and help local emergency workers.
Wisconsin Department of Natural Resources spokesperson Katie Grant did not immediately respond to an email asking if the derailment has resulted in any environmental contamination.
U.S. Rep. Derrick Van Orden, who sits on the House of Representatives Committee on Transportation and Infrastructure, said in a statement he was working with BNSF, the Federal Emergency Management Association, Wisconsin Emergency Management, the state and national Departments of Transportation and Crawford County officials to find out what occurred.
โMy staff is traveling to the site, and Congressman Troy Nehls, who Chairs the Transportation and Infrastructure Subcommittee on Rail, has also made staff available to assist our team," Van Orden said. "We will continue to monitor the situation and determine next steps.โ
Eileen Brown, who works at the Great River Roadhouse in De Soto, near where the derailment occurred, said, โWe have flooding right now and itโs causing business to be a little slow so Iโm kind of worried about whatโs going on down there.โ

While the ending of Donnie Darko has some dark implications, a deleted scene would have made the ending much less subtle and more gruesome. Filmmaker Richard Kelly initially captured a brief shot of Donnie, lying in his bedroom, still breathing and impaled by a piece of the plane. The shot emphasizes just how difficult Donnieโs sacrifice was, underlining the pain he went through to save everyone else.
Itโs also a much more horrific ending than the one the film ended up with. The scene is too graphic for the rest of the film. While Donnie Darko definitely dealt with some mature subject matter, it didnโt feature anything as explicit as that final shot. Itโs sad enough that Donnie sacrifices himself in the end. The deleted scene of him, lying in bed and dying, went a step too far.

That "being" knows what it is. I watched multiple videos of what happened. There was one man who did a video and was on the flight when it all happened. He said she was sitting next to a man and having a full on conversation. But it didn't register to him at the time that the guy never said a word to her. He stayed gazing at her though and she was talking. She spazzed and got up from the seat visibly disturbed making her way to the front of the plane. He looked back at the guy and he winked at him. But it wasn't a normal wink like a human with your eyelids shutting up and down. It was a wink with his eyelids shutting and opening from side to side like a reptilian. It caught him of guard and when the guy did get up to exit with everyone else he never said a word or acted phased by what had happened. I believe that woman and she wasn't drunk like they tried to say nor accusing him of stealing her air pods at that time. Her speech wasn't slurred, she wasn't stumbling or anything. If anything what she saw sobered her up quickly out of absolute fear.

Judicial Watch has sued the Department of Homeland Security (DHS) to uncover details about Bill Clintonโs trips on the โLolita Expressโ to โOrgy Island.โ The watchdog group said Tuesday they filed the lawsuit to obtain records of all Secret Service expenses incurred to provide โsecurity and or/other servicesโ to the former president during his trips to the private Caribbean island retreat owned by convicted sex offender and Clinton friend, Jeffrey Epstein.
Epsteinโs private Caribbean Island of Little Saint James was first exposed as โOrgy Islandโ in December 2014, when Virginia Roberts โ who is now a married, 31-year-old mother of three โ filed an affidavit in a Florida federal court alleging that when she was just 15 years-old she was procured as sexual entertainment by liberal socialite Ghislaine Maxwell for Epstein and his friends.
Judicial Watch President Tom Fitton said DHS failed to comply with a January 15, 2015, FOIA request, which named โany and all records reflecting expenses incurred to provide security and/or other services to former President Bill Clinton and any companions for trips to the Caribbean island owned by Jeffrey Epstein known as Little St. James from 2001 to the present date.โ
According to a flight manifest (logbook) โ viewable below โ Clinton flew aboard Epsteinโs private Boeing 727, dubbed the โLolita Express,โ on more than a dozen trips to โOrgy Island.โ Clinton, on at least one occasion was accompanied by โ4 secret serviceโ agents and Maxwell, the very woman prosecutors claim procured underage girls to sexually service Epstein and his friends.
โIf there is nothing to hide in the Epstein scandal, then why is the Obama administration breaking federal transparency law rather than giving us information about his travels?โ Fitton said. โThat weโve now had to go to federal court to try to get this Secret Service information speaks volumes.โ
Epstein pleaded guilty in 2008 and served just 13 months after Mrs. Roberts alleged he enjoyed the company of multiple teenage sex slaves. Roberts also put the former president at the scene, but only Epstein ended up a registered โTier 1โ sex offender.

โI remember asking Jeffrey, โWhatโs Bill Clinton doing here?โโ Roberts said in 2011. โThe former president, she added, was accompanied by four young girls during his stay โ two of whom were among Epsteinโs regular sex partners. And [Jeffrey] laughed it off and said, โWell, he owes me a favor.โ He never told me what favors they were.โ
Flight logs from the โLolita Expressโ during Clintonโs week-long anti-poverty and anti-AIDS tour, also listed the name Chauntae Davies for a period of five days. Davies, who appeared in Epsteinโs address book under an entry for โmassages,โ is a soft-core pornography movie actress.
โBeginning with his misuse of state troopers when he was an Arkansas governor,โ Fitton added, โBill Clinton has a long record of abusing his taxpayer-funded security details to facilitate and cover-up his illicit sexual activities.โ
Requests for comment put into both DHS and the Clinton campaign were not immediately returned. In the past, the Clintons have dismissed the questions surrounding Epstein, stressing that Bill severed ties with Epstein following his arrest in 2005. However, they have yet to provide an explanation for the flight logs.
The Justice Department says they believe that Epstein had upwards of 40 victims, in total.
